We are having the same problems with our relay server, and a similar configuration (DNS blacklist, rules, Groupshield). I've removed groupshield and the problem remains. I've put in the termination fix and removed pipelining, and the problem is still there. We downgraded our 2000 server to SP3 in case the SP4 update was a problem. All to no effect.
But the problem was not there last week. The only change in our configuration is that one client server moved from NT to windows 2000, but the Domino configuration was exactly the same. The smtp errors tho occur with sites outside the firewall, and only occasionally with the client server. Another client server with the same configuration shows no problem. Hard to see that this is the cause.
This is most puzzling, and if anyone has any ideas then please post!
